Help! I look so tired!

Do you feel like your eyes make you look more tired than you feel? If so, you’re not alone. This is such a common complaint I hear from patients and it’s because the under-eye area is one of the first places to show signs of ageing, stress, and lifestyle factors. As an aesthetics doctor, I frequently see patients who are concerned about dark circles, hollowing, fine lines, or puffiness beneath the eyes.

Tear trough filler is often the first treatment people think of. But what is it? And if it isn’t suitable for you, what alternative treatments are out there? Let’s get into it…

Why do we look tired under the eyes?

To begin, let’s look at why you might look tired. Before choosing a treatment, it’s important to understand the cause of your tired-looking under eyes. This could be from:

  • Volume loss (hollow tear troughs)

  • Thin or crepey skin

  • Lines which form due to ageing skin and repetitive movements in that area

  • Pigmentation or dark circles

  • Fluid retention and puffiness

  • Lifestyle factors such as poor sleep, stress, or dehydration, which can influence some of the above

A tailored approach to under-eye treatments is key. There is no one-size-fits-all solution and there may be a combination of problems resulting in the need for a combination approach to treatment.

What is tear trough filler and why might it not be for me?

Tear trough filler is a hyaluronic acid filler injected into the hollow area beneath the lower eyelid. This is a well-known option to treat the under-eye area, however it’s not always the most suitable treatment for everyone. In fact, I am trending away from filler in this area due to a possible risk of complications (such as swelling) and because it doesn’t always treat the underlying problem. Anyone with eye bags or swelling under the eyes, or general water retention in the area is not a good candidate for dermal fillers which can often (though not always) cause more swelling, sometimes months or years down the line.

The good news is that there is an array of effective alternative under-eye treatments that can help you look fresher, brighter and more rested.

Alternative treatments to tear trough filler

In a small number of patients, tear trough filler may still be beneficial. However, if you’re hesitant about filler or have been advised it may not be ideal for you, there are several effective alternatives to consider:

1) Polynucleotides

Polynucleotides are becoming increasingly popular in medical aesthetics, particularly for delicate areas like the under eyes. Rather than adding volume, they work by improving skin quality at a cellular level.

Benefits include:

  • Improved skin thickness and elasticity

  • Reduced fine lines and crepey texture

  • Brighter, more hydrated appearance

This is an excellent option if your main concern is skin quality rather than hollowing and it helps future proof the under-eye area.

2) Skin Boosters or Mesotherapy

Skin boosters and mesotherapy involve microinjections of hyaluronic acid or other active ingredients to deeply hydrate and revitalise the skin.

They can help:

  • Smooth fine lines

  • Improve skin texture

  • Restore glow and overall skin health

Unlike filler, skin boosters and mesotherapy do not significantly alter facial structure, making them a more natural option for patients wanting subtle rejuvenation.

3) PRP (Platelet-Rich Plasma)

Often referred to as the ‘natural’ treatment, PRP uses your own blood to stimulate collagen production. This treatment is going a little out of fashion in preference to polynucleotides.

This treatment is particularly helpful for:

  • Dark circles

  • Thin, dull skin

  • Early signs of ageing

PRP is ideal for patients seeking a regenerative approach without synthetic products.

4) Medical-Grade Skincare

Never underestimate the power of good quality skincare. This is a must if you’re concerned about your under-eye area. A doctor-led approach ensures you’re using evidence-based products tailored to your concerns.

Key ingredients may include:

  • Retinol (to stimulate collagen)

  • Vitamin C (to brighten pigmentation)

  • Peptides (to support skin structure)

Daily SPF 50, sunglasses and a hat will help protect the under-eye area from further damage. Consistency here can significantly improve and protect the under-eye appearance over time.

5) Energy-Based Devices (e.g. Radiofrequency or Ultrasound)

For patients with mild skin laxity (early loss of skin elasticity), non-invasive devices can tighten and firm the under-eye area.

These treatments:

  • Stimulate collagen production

  • Improve skin firmness

They are particularly useful for those not ready for injectables.

6) Botox for Under-Eye Concerns

Botox can be used carefully in the under-eye area to soften fine lines and improve the appearance of ‘crepey’ skin. In some cases, small amounts may be used to relax the muscle responsible for dynamic wrinkles just below the eye (often referred to as ‘jelly roll’ Botox).

Botox may help:

  • Smooth fine lines when smiling

  • Reduce bunching of the lower eyelid

  • Create a more refreshed appearance

However, it is not suitable for everyone. Inappropriate use can worsen puffiness, so it’s essential this treatment is performed by an experienced doctor with a deep understanding of facial anatomy and after a thorough consultation.

7) Surgical Options (Blepharoplasty)

For patients with more significant under-eye bags, excess skin, or structural changes, non-surgical treatments may have limitations. In these cases, surgery can provide a more definitive and long-lasting solution.

Lower eyelid surgery (blepharoplasty) can:

  • Remove or reposition fat pads causing under-eye bags

  • Tighten loose skin

  • Improve overall contour of the under-eye area

While surgery is a bigger commitment, results can be transformative for the right candidate. A consultation with a qualified oculoplastic or plastic surgeon is essential to explore this option safely.

Choosing the Right Treatment

The best under-eye treatment depends on your individual anatomy, concerns, medical history and goals. As a doctor, my role is to assess not just the under-eye area, but the whole face, ensuring natural and balanced results.

In many cases, a combination approach delivers the best outcome, for example, pairing polynucleotides with skincare and Botox.
